Redesigning insurance claim form

A 2 weeks project in September 2021 (in Chicago)

Based on the information provided, organizing and designing the form for Blue Cross Blue Shield Association

Course - Introduction to Visual Communications

This course provides the fundamentals for conveying visual information in a way that is effective and clear. Through a deep understanding structure and context of content, we learned to apply visual elements, techniques, and principles crafting effective visual messages.

For this assignment, we were tasked to organize the word-based document of a list of questions asked when claiming the payments from an insurance company. Our mission was to make the form understandable for users by using grids, visual hierarchy, line spacing etc.

Intent

I had two intents when I designed this form. One is organizing the information so the customers can quickly skim through the document and identify where they should pay attention. Another is to show conditional questions visually. (Ex: If you answered yes, provide the following information) I explored the use color and visual elements(bands and arrows) to achieve my goals.

Approach

For the first iteration, I tried to digest all the information on the word document to determine if there were any redundant descriptions and directions. Then I moved on to InDesign and tried to create my first draft without using any color or image. After getting feedback on the first draft, I applied colors and images to give it more context and visual hierarchy.
